Patient Forms

Upon entering our office, our receptionist will  welcome you as a member of our family.  We will request that you complete our  patient forms located in the New Patient Center. This paperwork provides us with your health history and information on your condition.

Consultation

Next, you will have a consultation with Dr. Christopher Wood to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Christopher Wood to learn more about you, your condition, and  expectations to determine whether you will be a good candidate for care.

Examination

After your consultation, Dr. Wood will perform a complete chiropractic examination utilizing standard neurological, orthopedic, postural and palpatory procedures. He may also wish to utilize more advanced diagnostic procedures, such as Surface Electromyography, Digital Postural Evaluation, or Computerized Foot Analysis.

Treatment

 At the conclusion of this initial  appointment, you can choose to  experience your first treatment. This may include spinal adjustments, physical therapy, and/or soft tissue therapies.  Dr. Wood  and his assistant will take time to thoroughly explain every aspect of treatment.

Your Second Visit

On your second visit Dr. Wood will take the time to visit with you prior to the initiation of treatment to see how you responded to your first treatment, to explain any test results available and to outline a plan for care.
